Last episode (S18E13) there was no sign of Maca or Esther. They took some days off (again) and Alicia signed to be “nurse in chief” as everybody encourages her and also she heard how Esther said she was thinking about quitting… She tries to speak to Esther but the cell is off.
This episode starts with Maca and Esther arriving at the hospital and Esther asking Maca if she’s going to go with her to “the book thing” that evening… Maca assures her she’ll try hard but she has to meet the lawyer for some inheritance reasons. Esther tells her she begins to be tired of that topic and it's taking Maca too much time and efforts.

Esther does not know about Alicia signing for the job. Alicia tried hard to meet and tell her but luck was not on her side and she is asked to assist in an operation before Esther arrives.
Some time later someone tells Esther that she must “work harder now”, obviously she has no idea what they are speaking about. (The thing is that nurses must vote and the winner gets the job).

Just then Reyes arrives… Maca is going to make her an ultrasound and they also speak about that evening and that finally the money is getting divided between all of them, including Reyes.

Later we see Esther asking Gimeno if he’s going to her book presentation that evening.

Esther: If it wasn’t for you there would be no book.
Gimeno: Well, I’m very busy but I’ll try… Look, that will be one of the advantages of Alicia getting your job, you’ll have more free time to write!
Esther: What are you talking about??
Gimeno: You are right, you’re going to win. Alicia is nice, very nice, but the nurses treat her as another nurse… but you… you have that thing called “autoritas” by the romans, natural authority!

She’s not taking it well…
Maca is with Reyes “and the baby”. Maca tells her they’re both getting the same amount of money and Reyes thanks her. Without her help she wouldn’t had the strength or the money to go on.

Finally Esther and Alicia meet…
Esther: Alicia, I was looking for you.
Alicia: Me too.
Esther: Well, here I am…

Alicia: Well, now I can’t.
Esther: Well, I’m still in charge, despite what some people think.

Does that mean (since I am trying to figure out how they will wrie these two out of the show) that Maca will open a cozy little pediatric office somewhere while Esther earns the big bucks as a children's book author?
Although I'd love another season of Esther putting Alicia in her place. Grumpy Esther rocks.
