This episode starts with Maca arriving to the hospital with her mother, Rosario. Esther goes to kiss and hug her wife (Maca’s been some days at Jerez with her mom and family).
It all seems normal to all, but this is Rosario’s face:

Esther: How are you Rosario?
Rosario: As you can see… like a rose.
Lots of tension in the air, they all notice it…
Maca tells Alicia about the test that her mother needs done but Esther, feeling responsible, says that she will take care of everything. Again, Rosario’s face:

Some time later Esther tells Maca that she has already done Rosario’s analysis…
Maca: You are just great, love…
And Esther says, just moving her mouth… and with a BIG smile while she goes away...
Esther: You owe me a kiss!

I would say Maca will pay her debt, hehe.
They meet again and Esther tries to tell Maca that the way her mother is acting is just unfair… Maca knows that Esther is right but also knows her mother so just asks Esther to stop telling her that because there’s no solution.

Esther: I know, but I can’t help it. I love you.
Maca: I know.
And Maca watches Esther go away, almost drooling.

Later we know that Esther goes on writing stories for the children.
Esther is again with Rosario, who’s complaining about everything…
Rosario: I don’t need a wheelchair and I don’t need you to take me anyway. I can go by myself to the X-rays.
Maca asks Esther to be patient, that it's just one day… Esther has to take a deeeeep breath.

Some time later Esther sees Rosario just walking around when she was supposed to be at some X-rays…
Esther: Rosario, have you already done the X-rays?
Rosario: No. (Without looking at her)

Rosario: I can’t find the X-ray room that you told me.
Esther: Come, I go with you.
Rosario: Look Esther, we’re both too grown up to pretend we get along. Maca is my daughter but you ARE NOT, so… it's not necessary to go on with this game.

...quick, part two, before I end up killing Rosario!!
She acts likea spoilt brat and seems to live off hurting others. Points to Esther for remaining polite (and for the cute hairdo). Also, Maca: drop that construction site of a mother and focus on your wife some more. That's it.
The cute couple expressions make it worthwhile. Just cut out the blonde.
